Services

Home care services are operated through licensed and Medicare-certified offices. Skilled home health includes Registered Nurses, Licensed Practical Nurses, Licensed Vocational Nurses, Physical, Occupational and Speech Therapists, Medical Social Workers and Home Health Aides. These services can help avoid unnecessary hospitalizations, shorten hospital stays, and enable an individual to stay at home during acute illnesses.

Oak River Healthcare Services Inc. is a home health and community support service licensed by Texas Medicare ( A CHAP accredited), Skilled Nursing and private duty nursing, home health aide and patient assistant services, we provide ‘In Home Nursing Care‘.

As a full service Home and Community Support Health Care Agency, we provide professional licensed and non-licensed personnel for in-home services in the following areas:

SKILLED NURSING

Working in conjunction with a physician, a Registered Nurse will act as case manager of the patient’s home care. The nurse will come to the house to evaluate a patient’s needs and develop a plan of care. Through regularly scheduled visits to the home, the nurse, along with other members of a health care team, will attend to the patient’s clinical needs, provide teaching and support and coordinate delivery of additional services, as needed.

REHABILITATION THERAPIES 

  • Physical Therapy – A licensed Physical Therapist will conduct a comprehensive evaluation of the functional level and, in conjunction with a physician, will develop a plan of treatment. The treatment will be carried out through a schedule of regular in-home visits.

  • Occupational Therapy – A licensed Occupational Therapist, in conjunction other members of the home care team, will develop and implement an in-home treatment plan to retrain and rehabilitate essential life skills lost as a result of an illness or injury.
  • Speech Therapy – A licensed Speech Language Pathologist, working in conjunction with other members of the home care team, will assess the patient’s needs and develop a plan of in-home care that will improve communication and/or speech, swallowing and language difficulties occurring as a result of illness or disability.

PRIMARY HOME CARE (PHC)

In conjunction with skilled nursing care, home health aides are available to provide personal care, meal preparation and assistance with daily living activities.this is part of the programs offered through the Texas department of Human Services.

COMMUNITY BASED ALTERNATIVES

This program allows others to give up to 24 hour supervision to the client under certain conditions.This is an alternative to Nursing Homes and Personal Care Homes.

PSYCHIATRIC SERVICES 

Our Psychiatric Services is a program for patients requiring short-term, intermittent psychiatric/mental health care. Our professional staff members have many years of experience and extensive knowledge in psychiatric, as well as medical nursing.We provide quality care for patients with psychiatric, mental health and medical disorders.

Depending on the client’s needs, the treatment team also may include:

  • Home health aides
  • Medical social workers
  • Occupational therapist
  • Physical therapist
  • Speech therapist
